Requirements
- Project address, a description of the work to be done, and the valuation of the project.
- Applicant must identify whether they are the property owner or a contractor, and if an employee, whether of the owner or of the contractor.
- "Verification of Worker's Comp. Required (including Carrier, Policy # and Expiration Date, Name and Phone Number of Agent)" unless the applicant certifies "Exempt, No Employees".
- Property owner's name, address, phone number and e-mail.
- For a contractor, architect or engineer: company name, address, phone, e-mail, City License number, and State License or Registration number.
