- New single-family or two-family residential construction, moving a trailer/pre-built home onto a lot, or any addition, remodel, or demolition
- Complete set of plans drawn to scale, submitted to the Building Official for review (per the city's Plan Requirements sheet); plans for single-family residences generally need not be architect/engineer-stamped unless the Building Official deems it necessary given project scope/design
- Legal description and San Juan County property tax parcel number
- Building permit application filled out completely and signed by the owner or agent, listing all contractors/subcontractors with current Utah Contractor license (name, license number, licensed trades, expiration date), address, phone, and email
- Property must be deeded in the applicant's name and recorded in the San Juan County Recorder's Office before permit issuance (or a notarized statement from the legal owner granting permission to build)
- Owner-builders performing their own work on a personal, non-commercial single-family residence must submit the Owner Build Certification and Agreement prior to beginning construction, per the Utah Construction Trades Licensing Act; all commercial project work must be performed by licensed contractors — commercial owners may not act as their own contractor in Utah
