- Maximum access width: 24 feet for agricultural/residential access; 40 feet for commercial, industrial, or subdivision roadway access
- Access surface, drainage structure (if required), and location must be shown on a plot plan on the reverse of the application
- Permit becomes void if construction has not started within 30 days of approval
- County Road Foreman must be notified 24 hours before construction begins; work must be completed within 90 days
- Applicant must call the County Road Foreman for final inspection and approval before the access is considered complete
- Applicant assumes responsibility for safe construction/maintenance and holds the county harmless for damages arising from the access
