  • Provide a completed CCOB Form Ensuring Compliance with Worker's Compensation Insurance Coverage Requirements (per SB26-093 / C.R.S. 8-41-213, included in the permit application); projects over $1 million total construction cost must also submit CDLE Form WC205
  • Provide a completed water license application with estimated water usage and supporting documentation if adding a significant number of new plumbing fixtures
  • Submit separately to North Metro Fire District, and separately to the Broomfield Health Department if the project has food service or a public pool
